Dr. Leslie Perry Collins, PT, DPT, MA

A native of eastern Kentucky, Dr. Collins is licensed to practice in Kentucky and Ohio. She completed her Doctor of Physical Therapy at the University of Kentucky in 2009, and was part of UK’s inaugural physical therapy doctoral class.
During that time, she received extensive training on treatments for cerebrovascular accident conditions at Cardinal Hill Rehabilitation Hospital as well as elective courses in Aquatics and Hippotherapy. She graduated with honors and was elected the Most Outstanding Physical Therapy Student by the UK faculty. Since graduation, she has enjoyed working in outpatient, home health, and long term care nursing facilities. Dr. Collins specializes in geriatric care and is popular for her manual therapy skills. She has been a member of the American Physical Therapy Association since 2006.
Dr. Collins grew up in the small town of Salt Lick, Kentucky, located in rural Bath County. She graduated from Bath County High School and has the distinct recognition of being named Miss Bath County 1994. She attended Morehead State University completing a Bachelor of Arts in Psychology and a Master of Arts in Higher Education. At MSU, she was a member of the MSU Equestrian Team and Delta Gamma Sorority. A back injury led her to physical therapy treatment and it changed the course of her career from counseling services to becoming a physical therapist.
Collins is an avid equestrian and enjoys showing the family’s Tennessee Walking Horses. She is a certified riding instructor and has won five world championships, two world grand championships, and multiple reserve world titles at the National Walking Horse Celebration held in Shelbyville, Tennessee, as well as World Versatility and International Grand Championships.
Dr. Collins lives in Maysville, Kentucky, with her husband Danny Collins. Danny manages the family farm, known as Locust Bend Farm, and they have two children, Ruth and Rebecca.
